Ascent Benefits Company makes the group enrollment process as painless as possible.  New group acceptance will beconsidered after Ascent Benefits Company receives the following:

• receipt of a a completed and signed application for group dental coverage
• return completed enrollments forms
• a deposit for the first month's premium
• a proposal outlining benefits
